Caregivers are quintessential for many individuals with advanced and chronic conditions to continue living in their homes. While some caregivers experience benefits of caregiving, such as becoming closer to the care recipient and being more present, the caregiving role can also be complex, difficult, and mentally and physically demanding. Additionally, a large body of evidence demonstrates associations between caregiving and poor physical and mental health outcomes. There are a rising number of federal, state, and local legislation that benefit caregivers. Federal legislation, such as the CARES Act, effective in 42 states, requires hospitals to incorporate caregivers’ contact information into the patients’ medical record and provide training for medical tasks they are to perform in the home. The Family and Medical Leave Act in California offers unpaid, job-protected caregiver leave. In-Home Support Services (IHSS) for Medi-Cal enrollees, provides caregiver support for hours dependent on need and also offers a way for caregivers to become an IHSS provider and receive compensation to assist their loved one. Finally, Veterans Affairs provides caregivers need-based stipends and educational caregiver training. While these benefits are proving effective for caregivers that receive them, not all caregivers are eligible for such benefits, and many do not have the resources they need. The article also highlighted the fact that while first launched to serve predominately African American adults, ACCA intentionally expanded the Advanced Illness Care Program (AICP) to serve the Latinx community by developing bilingual trainings and resources (in English and Spanish). In 2023, AC CARE Alliance served 162 participants in LA, San Mateo and Santa Clara counties, and a total of 387 people (including caregivers) across all six counties. AC CARE Alliance also conducted 1,500 participant visits, and continued to expand opportunities for learning and connection by training faith leaders, pastors, their congregants, and the broader community in the topic areas of aging, palliative care, hospice care and advance care planning. CAPC conducted a comprehensive environmental scan to better understand the health care experiences of Black Americans living with serious illness, and their families, and how to bridge disparities in access to palliative care services. The scan sought to identify opportunities to reduce these inequities, and ultimately uncovered hundreds of interventions that are moving the needle on health equity. AC Care Alliance was highlighted in CAPC’s Innovation Hub as a replicable solution making a breakthrough change in the care of serious illness and was featured in a 2023 article entitled “Inspiring Teams to Engage in Health Equity Interventions to Improve Care for Black Patients and their Caregivers” by Brittany Chambers, MPH, MCHES and Chelsea Perez. Arla’s daughter regularly visited with Ms. Joyce during her stay in the skilled nursing facility, being her advocate and keeping her company.
