Our Care Team members are caring, compassionate and trusted members of the community.
Many are dedicated members of one of our churches and have been identified for this work by their pastor, in partnership with ACCA leadership. They have all completed extensive training in our Advanced Illness Care Program™ and HIPAA compliance, and participate in ongoing trainings for continuous improvement.
ACCA Care Team members work closely with program participants to deliver a personalized Advanced Intervention Care Program in our 5 Cornerstones with the ultimate goal of empowering their participants.
