Pilot the Advanced Illness Care Program
with your Health Organization

 

Do you want to expand your serious illness care programs and provide wrap-around support to patients in your community? Partner with the ACCA to pilot our advanced illness care program with your palliative care or primary care clinicians

We can work with you and the appropriate payer to test the ACCA AICP with patients with medicare, medi-cal, dual eligibles or can customize an approach to meet your needs. ACCA can help customize an evaluation for the pilot in partnership with key stakeholders to plan for long-term sustainability.

Join the ACCA Network of Churches

Participate in community events, coaching and training sessions and Faith leader activities.

Refer congregation members for ACCA-AICP intervention.

Community Groups

If you are a community organization that provides resources to individuals with serious illness or caregivers and would like to partner with ACCA, please contact us.

We can explore opportunities to incorporate your organization into the AICP as a known resource recommendation for participants, once reviewed and assessed by ACCA leadership and navigators. We can also partner to establish a referral process of patients in your organization to ACCA that would benefit from the AICP.

Request Technical Assistance

The ACCA can be engaged to provide assistance in the following ways.*

  • Consultation and training for organizations that want to replicate the Advanced Illness Care Program. 

  • Community outreach partnership assistance bringing health organizations and faith communities together.

  • Structured support on how to use the Care Planning Workbook and other ACP toolkit tools and activities.

  • Conduct training with health organizations on:

    • ACP and using the care planning workbook with patients

    • Sharing best practices on discussing advance care planning with the faith-based African American community
