Care Navigators work with program participants to identify and address needs, provide trusted resources and encourage skills to empower persons needing advanced illness care and their caregivers in Five Cornerstones, all free of charge

Through a series of 5 to 12 in-person meetings and phone conversations over a period of about 6 months, the Care Navigator helps participants meet their advanced illness care needs and links them to resources.

Who Is Eligible

  • If you have an advanced stage illness (self-identified) and live in one of our service areas.

  • If you are a caregiver of someone living with an advanced-stage illness who lives in one of our service areas.

  • Currently we are able to provide service to residents of Alameda County, Contra Costa County, San Francisco, San Mateo County, Santa Clara County, and Los Angeles County.
