Sharing What We Have Learned

The work of the ACCA shows that the FAITH community is a TRUSTED resource for individuals with advanced illness and their family caregivers.

 

ACCA Care Navigators BRIDGE health system-community GAPS in care and successfully partner with health systems, community-based organizations, and other key health stakeholders on behalf of program participants.

Findings from evaluation activities, led by the UC Davis Betty Irene Moore School of Nursing, indicate program impact among individuals with advanced illness and caregivers, and impact on church congregations and the community as a whole.

Care Navigators are almost always able to form trusting relationships with participants.

High need for resources among persons needing care and caregivers

This was found in all program cornerstones (physical and mental health, spiritual, social, advance care planning, caregiving). Resources and referrals are provided by care navigators to meet these needs.

  • 3.5 average social referrals per participant

  • Caregiver needs addressed at 53% of visits

Participant empowerment and enhanced communication skills with clinicians

  • Goal setting at 100% of visits

  • Prayer at 85% of visits

  • Training with the ACCA Clinician Visit Tool at over 35% of health cornerstone visits

Advanced Care Planning Milestones Met

  • 55% of participants completed an advance directive (compared to 15% national average)

Trained care navigator workforce

  • low reported work stress in the face of challenging caseloads

  • Strong pastoral support for the program from an engaged team who genuinely want the program to succeed.

Congregation Assessment Survey Results

Developed in collaboration with our evaluation team at UC Davis, the ACCA developed a short Congregation Assessment Survey to help the ACCA and church faith leaders better understand a congregation’s needs related to advanced or serious illness and caregiving.

2019

Conducted across ACCA churches every few years and was completed in 2018 and 2019. In 2019, 643 surveys were collected from 14 churches.

The duration of an individual church’s involvement with the ACCA program and placement of a navigator within a church was associated with higher rates of advance care planning milestone completion within members of that congregation.
